ALL UPPER SCHOOL ATHLETES must complete the following items before participation in athletics is allowed: 1. Physical and Medical History Form uploaded to Magnus Health Physicals are valid for a year from the date of the examination. You can access Magnus by clicking on the Magnus Tile on the resource board once signed in at https://smcprep.myschoolapp.com/app/school-website#login. 2. TMS Student and Parent/Guardian account - updated profile and acknowledgement of rules TAPPS requires both students and a parent/guardian to have a TMS profile. To complete this requirement, the student must log in first using their SMP email, update their profile, and check off the acknowledgement of rules. Once they add their guardian's email, the guardian may log in. If the email address is not listed under the student's profile, you will have to wait for approval.
